As a parent, family member or carer, you may be able to access GP services on behalf of another person. This is known as Proxy access.
Proxy access can allow you to manage appointments, prescriptions and, where appropriate, access medical records for someone else. The level of access granted will depend on the patient’s consent and circumstances.
How to Request Proxy Access
You can:
- Collect a Proxy Access Form from reception during our opening hours.
- Download and complete the appropriate form below and return it to reception:
Linked Profiles in the NHS App
Once proxy access has been granted, you may be able to access the other person’s profile through your NHS account using the NHS App or NHS website. Find out more about using linked profiles in the NHS App.
Important Information
We cannot share information about an adult patient without their explicit consent, regardless of their relationship to the patient.
Proxy access for children is reviewed as they get older and may be amended or removed in line with NHS guidance, confidentiality requirements and the young person’s ability to make decisions about their own healthcare.
If you are unsure which form you need, please contact reception and a member of the team will be happy to help.
