Registering with the surgery
You can register as a patient by either
- Clicking on the NHS Register with a GP surgery and completing the registration form online.
- Printing off this form and emailing to ddicb.woodville-surgery@nhs.net or handing into our reception desk.
- Collecting a paper registration form in person from our reception desk.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
